Our Mission and Vision
The Eastern Ottawa Community Family Health Team provides, primarily to francophones, primary health care for overall health.Together, attentive to health care needs, the EOCFHT improves the well-being of everyone.
The community benefits from a range of health services that meet the needs of users. We thrive on strenghtening our organizational and efficiency capabilities to maximize our resources to ensure the quality of our services. Improvement of our processes (clinical, administrative and financial) and infrastructure redevelopment contribute to the quality of our services.

As per the Ministry of Health and Long Term Care of Ontario: "A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient ".
Our dynamic team is formed by 10 family doctors, 4 registered nurses and pratical nurses, 4 nurses practitioners and other health care professionnals such as dietician, pharmacist, social workers and psychologist who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible.
