Since 2015, The EOCFHT is proud to offer a memory clinic close to home for our patients.
This service is offered to patients with a cognitive deficiency or having notice over time a change in their capacity to accomplish acitvities of daily living.
During the initial visit at the memory clinic, you will encounter a multidiciplinary team composed of a nurse, a nurse practitioner, a social worker, a pharmacist and the family physician. This team has a advanced training in memory functions and functional capacities evaluation, such as car driving capacity. Once the evaluation is completed, treatment recommandations and available ressources will be discussed.
You should planned 2 to 2,5 hours for an inital assessment at the memory clinic, this time is essential for a complete and thourough evaluation.
A referral is needed to access this service. You are encourage to discuss with your family physician or the nurse practitioner.
This program is based on the memory clinic model for primary health care, elaborate by Dr. Lee and supported by the ARTIC initiative, who is well establish Ontario wide.
