Important notice regarding registration with our team
We understand that some recently published information may have suggested that the Eastern Ottawa Community Family Health Team is currently accepting new patients or maintaining an internal waitlist.
We would like to clarify that we are not accepting new patients at this time and that we do not maintain an internal waitlist.
Anyone looking for a family doctor or nurse practitioner must register with the provincial Health Care Connect / Accès Soins list. During registration, you may indicate your language preference if you wish to receive services in French.
Please do not call the clinic or send emails about this matter. No further information is available at this time. Calls and emails about this issue reduce our ability to manage daily clinic operations and serve our current patients.
We understand that finding a family doctor can be difficult and frustrating. Thank you for your understanding

Our approach
Family Health Team
The Eastern Ottawa Community Family Health Team (EOCFHT) provides, primarily to Francophones, primary health care aiming overall health. Attentive to health needs, the EOCFHT staff strives to enhance everyone's well-being by working together.
The community can benefit from a wide range of health services to meet their needs, as patients of EOCFHT. We rely on the competencies, efficiency and organizational skills of our staff in order to maximize the use of resources available to them. Improving management processes and redesigning our infrastructure - through renovations, relocations, etc. - are just a few examples of the means used to maintain the quality of our services.
Our specialized programs
The East Ottawa Community Family Health Team offers a variety of programs and services designed to meet your needs.
The EOCFHT primarily provides primary health care. We value a targeted, individual and/or group approach to promote, influence and support behavioral changes related to lifestyle habits and/or biological factors.
In an effort to support our patients in their overall health, both physical and mental, the EOCFHT offers mental health services consisting of assessment, psychotherapy, counselling, education and navigation of community resources. These free, short-term services are provided by the team made up of social workers, a community health worker and a psychologist.
The East Ottawa Diabetes Education Program is a proud partner of Diabetes-Ottawa. This program is for anyone diagnosed with diabetes, whether or not they need an endocrinologist. The specialized team, made up of a registered nurse and a dietitian, educates and supports adults with prediabetes or type 2 diabetes, as well as their families.
