Family Health Teams focus on the well-being of the patient and provide access to many primary health care services offered by the professional members of the team. The goal is to maintain optimal health, manage chronic disease and maintain quality of life.

The East Ottawa Community Family Health Team receives patients registered with its doctors byappointment only in order to provide care to the best of its abilities.

The CFEO does not have the resources to provide emergency services. However, you may be able to see a doctor or nurse practitioner the same day or the day after your call. Each day, time pages are reserved for more urgent cases (sinusitis, otitis, urinary tract infection, etc.). If you cannot make it to your appointment, let us know as soon as possible.

Prepare for your next visit

Your visit to the doctor is always important, here are some tips that will help you get the most satisfaction from your visit and help the doctor or nurse practitioner (NP) serve you better.

  • Arrive 15 minutes before your scheduled appointment time. Forms will have to be completed before the visit, thus maximizing the interaction time with the healthcare professional.

  • Prepare a short document that will summarize your health questions so that you don't forget anything. Obviously, depending on the length of the meeting, the subjects must be discussed in order of priority.

  • Always bring the most recent version of your medications (pumps, pills, liquids) in their original containers provided by the pharmacist. Also include medications taken without a prescription. It is essential that everything is checked regularly to ensure the safety of taking your treatments.

In general, there are 2 types of meeting with the doctor or the IP. 

One-time visit: 

Usually, this follow-up is short-lived and usually geared toward a particular problem.

To help make an accurate diagnosis of the problem or discomfort encountered, you can answer these questions to further help your doctor:  

  1. How long have I had this discomfort?

  2. How long is   the episode? 

  3. Is it constant or intermittent

  5. Is it related to a specific event

  6. How do you describe the pain ? Its intensity?

  7. Does it spread to other places? if yes, where? When? 

  8. Have you consulted before for this same problem? 

Periodic visit: 

This is usually a visit linked to a periodic medical examination (formerly known as an annual examination).

In addition to the description of discomfort or problem above, other topics must be addressed such as:

  1. Any significant health problem occurring in your genetically related relatives (mother, brother, sister, cousins, grandparents, children)  giving the name of the disease.

  2. If relevant, bring readings of blood sugar, blood pressure, headache, etc. 

  3. Bring your vaccination record.
