Already enrolled in the program? 


Bring the following items with you to your follow-up appointments: 

  • Blood glucose log;

  • Glucometer;

  • Up-to-date list of your medications;

  • Results of your latest blood pressure readings.

Also, for an effective follow-up appointment, it is essential to do the following actions before your appointment: 

  • Blood test, 1 to 2 weeks before the appointment;

  • Take blood sugar regularly the week before the appointment;

  • Make a food diary the 3 days before the appointment. 

Are you diabetic?

The East Ottawa Health Team Diabetes Education Program is a proud partner of Diabetes-Ottawa. This program is for anyone diagnosed with diabetes. The specialized team, a registered nurse and a dietitian educate and support individuals and families affected by prediabetes or type 2 diabetes.

The services offered are:

  • Diabetes patient education in individual session

  • Diabetes management

  • Awareness and support during the achievement of objectives

Joint visits with the ETEFCO practitioners and the diabetes team contribute to our multidisciplinary approach, which reduces hospital visits and complications caused by diabetes.
